Please use the following forms to complete your student's registration at Sacred Heart Catholic School. For your convenience, we have provided a "Check List" for New Students Enrollment and Continued Student Enrollment. It is recommended that you review the check list FIRST. Please use the following medication forms for your student - as needed. These forms need to be on record in the school office by August 1st of the upcoming school year. Also, they may be need to be reviewed or completed again as medications or care plans change throughout the school year. Thank You!
